We know that living with a physical health condition can be challenging and stressful at times. This may be because the health condition itself can be more demanding to manage and the condition may be having significant impact upon many different aspects of your life including routine activities, mood, energy, sleep, and relationships. More than 15 million people in England, about 30 percent of the population – have one or more long-term health conditions which includes people who have Diabetes, Heart Conditions, Musculoskeletal difficulties, Chronic Obstructive Pulmonary Disease (COPD), Chronic Tiredness, Irritable Bowel Syndrome and Chronic Pain (including neuro-fibromyalgia).

Research suggests that the people can develop skills to manage their own long-term condition in partnership with health care professionals, which can have significant benefits on your health, quality of life and your emotional wellbeing.
